(Fracture Evaluation and Management Utilising Recovery data in Children)
The audit will assess the management of closed femoral fractures in children aged 0-5 years, focusing on treatment modalities, effectiveness, healing times, weight-bearing progression, complications, and adherence to NICE guidelines (NG38). For initial pain management in children under 16 with suspected long bone fractures, recommended options include oral ibuprofen and/or paracetamol for mild to moderate pain, and intranasal or intravenous opioids for moderate to severe pain, with consideration of femoral nerve or fascia iliaca blocks for displaced femoral fractures. All skeletally immature children with femoral shaft fractures should be admitted, with treatment based on age and weight: padded splints for birth injuries, Pavlik’s harness or Gallows traction for infants up to 6 months, Gallows traction for 3-18 months (if under 15 kg), and straight leg skin traction for ages 1-6 years (practical only for children under 25 kg), with possible conversion to a hip spica cast for early discharge.
